New medical data and clinical warnings link smoking and vaping to increased postoperative recurrence of Crohn’s disease and a higher risk of contracting pneumococcal pneumonia. These findings urge healthcare providers to manage alternative tobacco users with the same clinical caution as traditional smokers.

Crohn’s Disease Recurrence and Alternative Tobacco

A study published in the American Journal of Gastroenterology analyzed 937 patients who underwent ileocolic resection for Crohn’s disease. Lead author Dr. Tommaso Lorenzo Parigi from IRCCS Ospedale San Raffaele in Milan and his colleagues tracked patients for one year post-surgery to measure endoscopic recurrence.

The researchers observed that patients using heat-not-burn tobacco (HNBT) and electronic cigarettes experienced higher recurrence rates compared to nonsmokers. This association remained strong for HNBT users even in multivariable regression analyses.

“Based on these observations, we suggest that patients with CD using e-cigarettes or HNBT be managed similarly to traditional smokers in the postoperative setting,” the authors wrote.

How Vaping and Smoking Impair Lung Defenses

Beyond gastrointestinal risks, smoking and vaping weaken the respiratory system’s defenses against pneumococcal pneumonia, a lung infection caused by Streptococcus pneumoniae bacteria.

Dr. Sanjay Sethi, chief of the division of pulmonary, critical care, and sleep medicine at the University at Buffalo, explains that the lungs rely on specialized immune cells called macrophages to destroy invading pathogens. Smoking and vaping irritate the airway lining and damage cilia, the tiny hair-like structures that sweep mucus and bacteria out of the lungs.

“Anything that stresses the lung’s defenses gives an infection more room to settle in,” warns Dr. Jonathan Parsons, a pulmonologist at The Ohio State University Wexner Medical Center. This vulnerability extends to individuals with chronic conditions like COPD, asthma, diabetes, and heart disease.

Prevention and Vaccination Protocols

To mitigate these risks, the Centers for Disease Control and Prevention (CDC) recommends pneumococcal vaccination for all adults aged 50 and older. The recommendation also covers adults aged 19 to 49 who smoke cigarettes or manage chronic conditions like diabetes and asthma.

Dr. Parsons notes that while the vaccines do not prevent all mild respiratory illnesses, “they substantially lower the risk of serious complications like bloodstream infections, meningitis, hospitalization, and death.” Quitting smoking and vaping remains a primary recommendation to eliminate ongoing airway irritation.
